        PRIVACY ACT REQUEST INSTRUCTIONS  ( INFORMATION ABOUT YOURSELF)

·         Provide your complete name and mailing address.

·         Provide your date and place of birth.

·         You must sign your request and have it notarized.  In lieu of a notary, you may state “Under penalty of perjury, I hereby declare that I am the person named above and I understand that any falsification of this statement is punishable under the provisions of Title 18, United States Code (U.S.C.), Section 1001 by a fine of not more than $10,000 or by imprisonment of not more than five years, or both; and that requesting or obtaining any record(s) under false pretenses is punishable under the provisions of Title 5, U.S.C., Section 552a(i)(3) as a misdemeanor and by a fine of not more than $5,000.”

·         Additional information consisting of Social Security Number, former addresses is voluntary.  However, this information may assist the FBI in locating the records you are seeking.

·         If your request is limited to information about yourself regarding a particular incident, please provide as much detail about the incident and the time frame.

 

       FREEDOM OF INFORMATION ACT (INFORMATION ABOUT ANOTHER INDIVIDUAL, INCIDENT,  HISTORICAL  EVENT, BUSINESS, ORGANIZATION)

  • Provide your complete name and mailing address.
  • If your request is for information about a third party, you must submit his/her notarized authorization permitting the FBI to release information about that person to you.
  • If your request is for information concerning a deceased individual, you must provide proof of death.  The FBI will accept a death certificate, obituary notice, or Social Security Index page.  You should also submit as much identifying data about the deceased person as possible to assist the FBI in locating the information you are seeking.
  • Requests for Historical Events, Businesses, Organizations and other incidents, should contain as much information as possible in order to help the FBI locate the records you are seeking.

         HOW TO OBTAIN A COPY OF YOUR  FBI IDENTIFICATION RECORD / RAP SHEET 

 

  • Submit written request via the U.S. mail directly to FBI, Criminal Justice Information Services Division (CJIS), SCU MOD D2, 1000 Custer Hollow Road , Clarksburg , WV   26306 .  DO NOT MAIL TO LOCAL FBI OFFICE
  • Your request must be accompanied by satisfactory proof of identity consisting of name, date and place of birth, and a set of rolled-inked fingerprint impressions placed upon fingerprint cards or forms commonly utilized for applicant or law enforcement purposes by law enforcement agencies.
  • Each written request must be accompanied by a fee of $18.00 in the form of a certified check or money order payable to the Treasury of the United States .
